Nursing Home Transitions

Solutions for Independence educates people with disabilities about their choices for community-based living. We teach decision-making skills to people with disabilities and encourage  them to be their own advocates in order to acquire the skills necessary for them to live in the community. We also assist in identifying community resources and helping people with disabilities move back to the community with necessary support.

“I’m finally home.”

Ronald came to Solutions for Independence after suffering from a lumbar spine injury a few years before. He was put in a nursing home after the accident, but in no way was meant to be there. He had been enlisted in the army, worked for several corporately owned businesses, and was mentally and physically capable of living without the assistance of a nursing home. He was eager to get out of his current situation, and partnered with Solutions for Independence for help to begin living a fuller life. After carefully understanding his goals and capabilities,  we were able to move forward to further assist him to living independently. We put together a financial strategy for spending and partnered with a local non-profit called Greenest to begin filling his new apartment with furniture and decor.


Solutions for Independence assists people with disabilities at-risk of entering institutions to remain in the community. We work with the consumer in developing an Independent Living Plan that will increase the consumer’s chances to remain in or transition to a community–based setting.

Youth Transitions

Solutions for Independence provides guidance and support for successful transition for young persons with disabilities from the school environment to individual educational, employment, recreational and housing goals. We also give information to schools and parents to better educate young people with disabilities on how to transition successfully. We offer support, mentoring and leadership opportunities.
